About the position

The Nurse Practitioner at CenterWell Senior Primary Care plays a crucial role in delivering high-quality, value-based care to senior patients. This position is part of a multidisciplinary team that focuses on the physical, emotional, and social wellness of patients, ensuring a comprehensive approach to healthcare. The Nurse Practitioner will evaluate and treat patients, coordinate care services, and participate in team meetings to enhance patient outcomes and service delivery.

Responsibilities

  • Evaluate and treat center patients in accordance with standards of care.
  • Assist in the coordination of patient services, including specialty referrals and home health care.
  • Participate actively in daily huddles with the care team.
  • Complete medical record documentation in a timely manner.
  • Meet with the Regional Medical Director to discuss quality of care and outcome data.
  • Participate in potential growth opportunities for new or existing services within the Center.
  • Participate in the local primary care 'on-call' program.

Requirements

  • Minimum of two to five years of directly applicable experience preferred.
  • Successful completion of a Nurse Practitioner program with Board Certification.
  • Active, unrestricted Nurse Practitioner license in the state of practice location.
  • Excellent written and verbal communication skills.
  • Basic computer skills, including email and EMR.
  • Certification to write prescriptions under the authority of a collaborating physician.
