Position Description

 

The Mobile Integrated Health Program (MIH) is a collaboration between the Seattle Fire Department (SFD) and Aging and Disability Services, a division of the Human Services Department (HSD).   Case managers work alongside specially trained fire fighters who respond to clients in behavioral, medical or substance use crises. Our case managers are some of the most skilled, experienced and effective case managers in the business.  They are true experts in crisis intervention, service connections, and the clients who are referred to the Seattle Fire Department. The ability for our firefighters and case managers to work as a seamless unit is behind everything we have achieved in the program. 


The Mobile Integrated Health Program case managers respond to clients and conduct an on-scene assessment and plan of care, referring to services and supports to meet long term needs rather than the emergency room.  Health 99 is part of the MIH program. This is our post overdose response team and was created in response to the Mayor’s Executive Order to address the fentanyl crisis.  Health 99 case managers provide a trauma informed response to clients experiencing a substance use crisis and will follow-up to link clients to treatment or other services.


The MIH case managers will be employed by the Human Services Department and report to a supervisor within HSD and be embedded within the Seattle Fire Department. Initially the case manager will work a 4/10 schedule within Monday-Friday.  When the program expands its hours, this position may transition to some weekend hours.  This role will be in the office/field full time with the opportunity to have a hybrid schedule as permitted.  This recruitment will be used to fill multiple openings within the MIH program.

 

Job Responsibilities

 

The MIH Case Manager should be an experienced and proficient system navigator, with a working knowledge of medical, behavioral, substance use and social service resources throughout the city, as well as community-based organizations, and strategies for accessing them in real time for clients in need. 


  • Case managers respond to persons in medical, behavioral, or substance use crises and/or with social service requests alongside SFD firefighters in the Mobile Integrated Health vehicle.  
  • Respond to a high volume of dispatched and SFD system referrals and crisis situations daily. Case managers are expected to keep calm during daily crises responses and are often joined on scene by other crisis responders which can escalate the intensity.  
  • Perform an on-scene assessment and plan of care for each patient who may be agitated, upset, hostile, withdrawn, and/or difficult to communicate with. Due to the nature of a dispatched response, patients are not vetted or screened in advance. 
  •  Provide outreach, crisis intervention and short-term case management to individuals with chronic illnesses, behavioral health disorders, and substance use disorders. 
  •  Utilize multiple techniques for client engagement while working with difficult to engage populations.  
  •  Work effectively within diverse workgroups and diverse work cultures with proven ability to collaborate with first responders, community-based services, law enforcement, and governmental agencies.  
  • Navigate the mental health and healthcare system (including clinics, primary, urgent, and emergency care, hospitals, and insurance issues) for needy or underserved populations.  
  • Work in conjunction with the homeless services system of Seattle/King County to benefit our unhoused clients.  
  • Utilize de-escalation training and experience.

Minimum Qualifications:

  • Education: Bachelor’s degree in social services, Psychology, or a related field (or an equivalent combination of education, experience, training and/or work experience required). 
  • Experience: Three years’ social services experience involving interviewing, counseling, or crisis intervention.
  • Certification/License: Driver’s license for the ability to make on-site visits to clients and attend meetings and training.


OR: An equivalent combination of education and experience that demonstrates the ability to perform the position duties



Desired Qualifications:
  • Experience serving populations impacted by extreme poverty, behavioral health conditions, substance use and mental illness from a trauma informed and harm reduction model of care.
  • Lived experience of being unhoused or having a personal history of recovery from mental health, substance use or co-occurring disorders.
  • Certified Peer Counselor Certification a plus
  • Strong understanding of race & social justice principals and impacts of systemic racism and oppression on communities served by HSD.
Bilingual or multilingual a plus.

The full salary range for this position is $41.86 - $48.67 per hour.  Additionally, the Mobile Integrated Health Program case managers receive $5.00 per hour premium pay.
